§ 43-5. Contents of massage establishment license application.

  • At the time of making application for a massage establishment license as required by this chapter the applicant or applicants shall furnish to the chief of police or the person so designated by the chief of police:

    (a)

    Names and addresses of all employees of such massage establishment.

    (b)

    Certificates evidencing that all employees have obtained a health certificate from a regular practicing medical doctor showing that such person employed, or to be employed, by such establishment is free from any kind of infectious or contagious diseases.

    (c)

    Names and addresses of all persons or legal entities owning and/or controlling said applicant, including, but not limited to, all partners, principals or managers and also, in the event the applicant is a corporation, then the officers, directors and shareholders holding at least ten (10) percent of the said corporation corporate stock.

    (d)

    Disclosure as to all employees of applicant and any and all partners, managers or principals of applicant and also, in the event applicant is a corporation, all officers, directors or stockholders holding ten (10) percent or more of the capital stock of applicant, of all final convictions occurring within five (5) years of the offenses of sodomy, procuring, pandering, keeping a bawdy house, keeping an assignation house, engaging in prostitution, engaging in assignation or of any offense contained in Sections 43.02 through and including 43.05 of the Texas Penal Code of 1974.
