§ 43-6. Contents of massaginist license application.

At the time of making application for a massaginist license as required by this chapter the applicant shall furnish to the chief of police:

(a)

Applicant's present name, surname and married name, all names while married if applicant is divorced, address, telephone number, date of birth and Texas driver's license or if no Texas driver's license, department of public safety identification card.

(b)

A health certificate from a regular practicing medical doctor showing that applicant has been examined and is free from any infectious or communicable disease.

(c)

A statement listing all final convictions of applicant in any court occurring within the preceding five (5) years of the offenses of sodomy, procuring, pandering, keeping a bawdy house, keeping an assignation house, engaging in prostitution, engaging in assignation or any offense contained in Sections 43.02 through and including 43.05 of the Texas Penal Code of 1974.

(d)

Fingerprints and photograph of applicant made by the Irving Police Department which shall be attached to the application.
